Participation is voluntary and may include educational materials, creative activities, community events, and communication from our team or approved partners. 2. Purpose of This Consent Form Because Yott Kids Foundation may collect limited information from children under 13 years of age, we are required under the Children’s Online Privacy Protection Act (COPPA) to obtain verifiable parental consent before collecting, storing, or using any child’s personal information. This form provides consent for: Collecting your child’s name, age, and parent/guardian contact information. Communicating with you and/or your child for program participation. Sending information about activities, newsletters, and foundation updates. Using photos, artwork, or testimonials (if you grant permission below). 3. Parent/guardian’s full name, phone number, and email address. Mailing address (if materials or participation kits are requested). Responses to program activities or surveys.
